外国人验证须知Requirements for certificate confirmation
Foreigners who had a physical examination ahead of arrival may apply a certificate confirmation, please read the following instruction before you start the application.
Requirements for certificate confirmation
1. The physical examination record should be finished and issued in a governmental hospital abroad. If it was issued by a private hospital, the record should be notarized by a proper authority/notary. These examination records, whether issued by governmental or private hospitals should be confirmed by the nearest embassy or consulate of P.R.C abroad. When registration, the confirmation letter or label for the record is needed.
2. The results of the physical examination have to be filled in the form named as “Physical examination record for foreigners”, and have to be attached all original reports. Two stamps are needed, one is crossed on photo of the applicant at the first page, and the other should be on signature of doctor’s at the second page. Date of the report at the second page should be clearly. These physical examination records are valid for six months from the date of issue.
3. Before registration, applicants should prepare following documents:
l Original papers of the “Physical examination record for foreigners” and attached reports.
l The confirmation letter or label for the record.
l Passport, passport copy (information page and the latest visa page) and 5 photos.
4. If the records met the requirements above, confirmation certificate will be issued in 4 working days.
Notice: Because there may be some technical and policy differences, acceptance of the physical examination record should be depended on our consulting doctors. You may be required to take some tests for the suspicious results or items you didn’t finish abroad in order to complete the confirmation. Thank you for your cooperation!
